Stephanie Bender-Kitz has been selected to participate in the Practice Change Leaders for Aging and Health program (www.changeleaders.org). The Practice Change Leaders for Aging and Health is a national program to develop, support and expand the influence of organizational leaders who are committed to achieving transformative improvements in care for older adults. The Practice Change Leaders program is jointly supported by the Atlantic Philanthropies and the John A. Hartford Foundation. Stephanie’s work convening the Idaho Community Initiative on Advance Care Planning, soon to be known as Honoring Choices Idaho, in collaboration with both major hospital systems, Idaho’s health insurers, Boise State University and community partners, is the perfect project for this national recognition.
